Default 94 4.0 automatic Slipping out of 4H when going 20-30

Okay, so first time going 4h for a long period of time above 20mph. Basically, it'd drive fine, and then slip and start making the ratchet noise. I'd throw it in neutral, switch back to 2wd, back to 4H, and then it'd drive fine for a second, and then slip again.

Anyone know what would cause this?

There are several things that could cause this, listed from easiest to most difficult to remedy:

1. Misadjusted linkage
2. Change (coins) stuck in the lever under the console (I've actually seen this happen)
3. Bad engine/transmission mounts and or TC drop
4. Bent or broken shift fork in TC
5. Worn pads on TC shift fork.
6. Stretched TC chain which can flop around and strike the shift fork at higher speeds, causing it to disengage.
