I've seen a lot of guys mount it on the side of the tranny tunnel, where the passenger sits, up near the underside of the dash. Smaller ones can be fit underneath the dash near where your right knee would sit.

It depends. Range really depends on the terrain you're in, line of sight, etc. If you have a clear line of sight (mountain top to mountain top), you can see upwards of 20 miles. In the woods, you're looking at 1-5 miles, depending on the antenna you've got and its placement on the vehicle.
Antenna length is technically infinite; CB's work best with an antenna greater in length than 8 feet, which is accomplished by means of a metal whip instead of a stiff fiberglass rod. I just installed a fiberglass 5ft. antenna on my Jeep, on the panel right above the rear passenger side tire, and it doesn't stick up above the Jeep very far.

If you have an aftermarket steel rack up top I would HIGHLY recommend putting the whip up there. You only need about a 3' one on a spring. I tuned my CB and its almost perfectly tuned. I can get out quite a ways and receive from quite a ways away. Just my opinion.
