Found a new issue. May be related may not be. If it is, hopefully it'll give one of you fine people an idea as to what to look at.
I'm getting a heavy "clunk" sound when I go from Drive to Reverse.

Push your TC tail up really hard, if you see the whole engine-tranny-tc assembly going up, the tranny mount rubber on the crossmember is shot.

Might not be your problem, but on my '97 ZJ w/5.2L I had that clunking and other issues. I could move rear drive-shaft at rear yoke up/down slightly at diff. housing. Turned out it was bad pinion bearings, carrier bearings, and the ring & pinion gears had to be replaced. Hope it's not your problem.

You said you were getting a clunck going from R to D and back right? really make sure that you dont have bad tranny and motor mounts.Just because they look good dosn't mean squat.

Seem to be getting closer to the problem.
The vibrations are getting worse
Now it's vibrating even at very low speeds. From stop, release the brake all the way and it starts vibrating at around 10mph, all the way through to about 30mph, then stops and starts again at 40mph then stops around 45mph.
Sooner or later something will break and fall off and I'll be there to fix it.

Well everything I read leads me to think your driveshaft is the issue, whether it's out of balance or even the splines are getting seized up. Many GM trucks had an issue where the spline on the yokes would get gummed up and cause vibrations and clunking as the yoke would not slide on the output shaft freely

I had similar problems with my 96 bronco. It turned out that all 4 bolts that held the drive shaft to the transfer case had loosened and one had completely fallen out! Earlier last year I would get a delay between dead stop and go when I gave it gas and that was fixed by greasing the slip joint.
